The Dare County Department of Health & Human Services- Public Health Division is seeking a self-starter, assertive leader to manage and provide services for the Diabetes Smart Education program. Responsibilities to include Diabetes Self-Management Education to individuals and groups. Community Outreach with Hospitals, community medical practices, businesses. Active participation on the Healthy Living/Chronic disease Task force, Collaborates with providers to manage the entire continuum of care for diabetes clients. Considers client knowledge, skills, cultural preferences, health beliefs and preferred learning styles and adapts education to meet client needs. Uses motivational interviewing to engage clients learning. Education and Experience Requirements: Registered dietitian with active registration with the Commission on Dietetic Registration or Certified Diabetic Educator or Registered Nurse that is CDE eligible. 2 years minimum employment experience as a diabetes educator and managing all aspects in the continuum of care. Marketing skills, Proficient Computer skills, public speaking skills, excellent customer service skills a must.
