A Partnership for a Healthier
Dare County


Community Health Assessment resides in the Public Health Education Division of the Dare County Department of Public Health. Community Health Assessment is a core public health function which drives local public health planning. The Healthy Carolinians process is used to conduct the assessment in a comprehensive manner.

The process includes the formation of a community-based partnership to steer the process. Health Educator Supervisor, Kimberly Cholewinski, serves as Coordinator of Healthy Carolinians of the Outer Banks (HCOB).

HCOB is a partnership of over 30 local organizations working toward a healthier Dare County. Since completing a thorough community health assessment in 2006, HCOB is now mobilized into Task Forces to combat community health problems.
