. . . Meeting Your Needs
Our experienced and qualified staff provide services within the home,
working under the direction of a physician to develop a plan of care based on the
individual needs of the patient. The family is included in this process.
Skilled Nursing includes assessment,
intervention, plan and evaluation of care tailored to the individual patient's need.
Certified Nursing Assistants provides
assistance with daily living and personal care.
Physical therapy provides procedures,
evaluations, and diagnostic tests to improve mobility, strength and function to assess for
home safety.
Speech therapy provides assessment and
intervention for speech impairments and assists patients to communicate more effectively.
Occupational therapy designs
goal-oriented activities to promote functional independence with dressing, grooming, and
meal preparation. Activities may address school and work related needs.
Social workers address psychosocial
needs of the patient and their family.
Hospice provides services for the
terminally ill patient and their families. The hospice program is designed to provide
comfort and supportive services to persons with limited life expectancy, at home,
surrounded by loved ones.
