Duration:
  • 1 Hour Initial Assessment with Registered Dietitian or Registered Nurse
  • 9 Hours of Group Classes with Diabetes Team

Classes start on a regular basis.

Eligibility:
  • Must have been diagnosed with type 1 or type 2 diabetes.

Chronic Disease Nurse Navigator

Provides disease specific education, addresses barriers to care, promotes self-management and connects patients to community resources.  Meets with patients in one on one office visits.

Eligibility:

diagnosis of uncontrolled diabetes or Stroke 

 

Diabetes Prevention Program - Prevent T2

Participants meet in a group setting with trained lifestyle coach. Class helps individuals who have been identified as pre-diabetic make small lifestyle changes to prevent or delay the development of type 2 diabetes.

Duration: 

1 year

  • Meets weekly for the first 4 months (16 classes total)
  • Every other week for the next 4 months (8 classes total)
  • Monthly for the last 4 months (4 classes total)

New sessions begin quarterly!

Eligibility: 
  • Be 18 years or older
  • Be overweight (body mass index above 24, or 22 if you are Asian)
  • Have NOT been diagnosed with type 1 or type 2 diabetes
  • NOT be pregnant
  • In addition, you must meet ONE of the following criteria:
    • Have had a recent blood test in the prediabetes range, which includes one of the following tests and results:
      • Hemoglobin A1C: 5.7–6.4% or
      • Fasting plasma glucose: 100–125 mg/dL or
      • Two-hour plasma glucose (after a 75 gm glucose load): 140–199 mg/dL
    • A doctor previously diagnosed you with gestational diabetes.

 

Health & Wellness Coaching

The coach co-creates individualized action plans with each patient to support them in achieving their personal health goals. Encourages adoption of habits that are conducive to a higher quality of life.

Nutrition Counseling

Nutrition counseling for diabetes, obesity, weight loss, Hypertension, Cardio Vascular Disease, plant-based diets, weight loss surgery, and more. Will provide evidence-based nutrition advice, as well as individualize a plan that works best for each patient. Meets with patients in one on one visits.
